JOB DESCRIPTION:
Provides professional physical therapy services to Home Health patients under the supervision of a licensed physical therapist. Acts in a manner consistent with the State's professional practice Act for physical therapy as well as the organization's policies and procedures. Reports changes in patient status, confers with the supervising therapist, makes recommendations for alteration of the care plan.
EDUCATION
  • Associate's degree from an accredited physical therapy assistant program, required.
EXPERIENCE
  • Two years minimum clinical experience in an acute care, skilled nursing, or community/home health setting preferred.
LICENSURE/CREDENTIALS
  • Licensed as a Physical Therapist Assistant in the state of Washington, required.
  • Current BLS certification required.
  • Valid Washington State driver's license.
  • Proof of insurance on automobile used for work required.
  • Driving record in compliance with OMC standards currently in effect or as may be amended. See HR Policy Driving for OMC Business.
OTHER
  • OMHH issued work cell phone provided.
  • Must have computer internet connectivity (OMHH work laptop provided).
  • Must have reliable automobile transportation to travel to patient residences.
