To thrive as a Weekend Case Manager, you need a background in social work, counseling, or a related field, along with strong assessment and care coordination abilities. Experience with case management software, electronic health records, or documentation systems and a relevant certification such as CCM or LSW is often required. Excellent communication, problem-solving, and time management skills help you effectively support clients and liaise with multidisciplinary teams. These competencies are critical to ensure quality care delivery and smooth transitions for clients, even during weekend shifts when resources may be limited.
A Weekend Case Manager coordinates and oversees client care during weekends, ensuring continuity of services and support. They assess client needs, develop care plans, communicate with service providers, and address any immediate concerns. This role is common in healthcare, social services, and nonprofit organizations, where 24/7 support is needed. Weekend Case Managers help ensure that clients receive proper assistance even outside regular business hours.
As a Weekend Case Manager, you are responsible for assessing client or patient needs, developing care plans, coordinating services, and providing support to individuals and families during the weekend. You may handle new intakes, facilitate communication with healthcare providers or community resources, and help resolve any urgent issues that arise. Weekend shifts often require autonomy and quick decision-making, as you may have limited access to other staff or resources. This role is vital in ensuring continuity of care for clients and maintaining the flow of operations outside of standard business hours.

This position supports a Home Hospice program providing compassionate end-of-life care across the community. Care is delivered in multiple settings including:
Patient homes
Assisted Living Facilities (ALF)
Skilled Nursing Facilities (SNF)
Group homes
Independent living communities
Two hospitals for General Inpatient (GIP) hospice care
The hospice program supports approximately 105 patients daily.
We are seeking an experienced RN Hospice Case Manager (RNCM) to manage and coordinate care for hospice patients. The RNCM will provide clinical oversight, patient education, and coordination with interdisciplinary teams to ensure comfort and quality of life for end-stage patients.
Travelers with hospice or home health experience are preferred.
End-stage hospice patients
Care provided in homes and facility-based settings
Focus on comfort care, symptom management, and family support
RN Case Managers: 7
Caseload: 14-16 patients per RN
CNAs: 6
LPNs: Not utilized in this unit
Minimum 2 years of RN experience
Hospice or Home Health experience preferred
First-time travelers may be considered but experience is preferred
Active RN License
BLS Certification
Valid Driver's License
Auto Insurance (required for home visits)
EMR: Epic & Rover
IV Pump
Medication dispensing systems
Patient safety monitors (chair and bed alarms)
Home health nursing bag & trunk supplies
Travel within the community to patient homes and care facilities
