Aspirus At Home in IRONWOOD, MI is seeking a REGISTERED NURSE CASE MANAGER to join our field team!
The Registered Nurse, Case Manager is a professional who assumes the responsibility and accountability for the provision of assessment, planning, implementation, coordination, monitoring and evaluation of nursing care options and services provided to Home Health patients of all ages and their families to meet the patients' health needs through communication and available resources across the health care continuum in all settings. The Registered Nurse, Case Manager participates in the development of care plans in partnership with Home Health employees and volunteers, Aspirus, Inc. entities and community partners.
HOURS: This position is Full Time; 1.0 FTE or 72 hours per pay period. Day Shift. On call varied weekends.
Experience/Qualifications
  • Knowledge of patient care standards and practices normally acquired through completion of a Bachelor's Degree or Associate Degree from an accredited school of nursing with current Michigan nursing license.
  • Possesses an active Michigan RN nursing license.
  • Current BLS certification or obtained within 90 days of hire.
  • Valid driver's license required.
  • Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.
  • Previous experience working in home health or hospice and five years of broad based nursing experience with additional training or experience with the terminally ill beneficial.
