Hospice Registered Nurse (RN) Case Manager โ€“ Full-Time

JourneyCare Hospice is seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area.

Tired of high caseloads, frequent weekend rotations, and feeling stretched too thin?

At JourneyCare Hospice, we do things differently. We prioritize manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families.

We are looking for a Hospice RN Case Manager to:

  • Manage a dedicated hospice caseload as the primary RN Case Manager
  • Perform patient assessments, admissions, and individualized care planning
  • Deliver skilled nursing care focused on comfort and dignity
  • Coordinate care with physicians and interdisciplinary team members
  • Conduct routine visits and adjust care plans as needed
  • Provide end-of-life support, including family education and pronouncement
  • Maintain timely, accurate documentation
  • Supervise Home Health Aides in compliance with regulations
