RN Hospice Case Manager - Home Hospice

???? Burlington & Walworth, WI
???? Day Shift | Monday - Friday

This position supports a Home Hospice program providing compassionate end-of-life care across the community. Care is delivered in multiple settings including:

  • Patient homes

  • Assisted Living Facilities (ALF)

  • Skilled Nursing Facilities (SNF)

  • Group homes

  • Independent living communities

  • Two hospitals for General Inpatient (GIP) hospice care

The hospice program supports approximately 105 patients daily.


???? Position Overview

We are seeking an experienced RN Hospice Case Manager (RNCM) to manage and coordinate care for hospice patients. The RNCM will provide clinical oversight, patient education, and coordination with interdisciplinary teams to ensure comfort and quality of life for end-stage patients.

Travelers with hospice or home health experience are preferred.

    • Caseload: 14-16 patients per RN

RequirementsExperience
  • Minimum 2 years of RN experience

  • Hospice or Home Health experience preferred

  • First-time travelers may be considered but experience is preferred

Certifications / Licensure
  • Active RN License

  • BLS Certification

  • Valid Driver's License

  • Auto Insurance (required for home visits)
