Job Summary:

The RN Case Manager coordinates care and services of selected patient populations through the acute care episode and across the continuum to support an effective discharge plan. The RN Case Manager works collaboratively with interdisciplinary teams both internal and external to the organization to improve patient care by assuming a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources. The RN Case Manager utilizes standards, guidelines, and protocols for care delivery and incorporates data to continuously improve care and outcomes.

EXPERIENCE DESCRIPTION:

A minimum of 3 years of clinical nursing is required. 5 years of clinical nursing and case management experience is preferred. Experience in case management field including transitional planning, outcomes management and assessment preferred.

EDUCATION DESCRIPTION:

Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required. Bachelor's degree is preferred.

TRAINING DESCRIPTION:

Accredited case manager certification (ACM-RN) through the American Case Management Association (ACMA) preferred.

SPECIAL SKILLS DESCRIPTION:

General knowledge of payor industry (Medicare, Medicaid, & Commercial insurance regulations), resource management, and evidenced based clinical practice is essential. Theoretical knowledge of the nursing and case management processes. Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Advanced communication and interpersonal skills with all levels of internal and external customers including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and members of the community. Other skills needed include excellent written/verbal communication skills, critical thinking, creative problem solving, proficient organization and planning. Basic computer skills. Must be self-directed with ability to tolerate frequent interruptions and a demanding workload.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
