Job Summary:
The RN Case Manager coordinates care and services of selected patient populations through the acute care episode and across the continuum. SPECIAL SKILLS DESCRIPTION:

Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Other skills needed include excellent written/verbal communication skills, critical thinking, creative problem solving, proficient organization and planning. Basic computer skills.
