Case Manager โ€“ Substance Use Disorder (SUD)

Location: Lawrence, KansasJob Type: Full-Time

Position Overview

Avalon Wellness & Recovery Center is seeking a compassionate, organized, and recovery-oriented Case Manager to join our team. The Case Manager plays a critical role in helping individuals navigate the recovery process by connecting them with services, resources, and support systems that promote long-term success.

The ideal candidate will have experience working with individuals affected by substance use disorders and co-occurring mental health conditions, possess strong advocacy skills, and demonstrate a commitment to person-centered, trauma-informed care.

Key Responsibilities
  • Conduct assessments to identify client needs, strengths, barriers, and recovery goals.

  • Develop individualized case management plans that support recovery, stability, and community integration.

  • Coordinate care with counselors, medical providers, peer support professionals, family members, and community partners.

  • Assist clients in accessing housing, transportation, employment, healthcare, benefits, legal services, and other community resources.

  • Provide ongoing support, encouragement, and advocacy throughout the recovery process.

  • Monitor client progress and adjust service plans as needed to support successful outcomes.

  • Maintain accurate, timely, and professional documentation in the electronic health record (EHR).

  • Participate in treatment team meetings and contribute to interdisciplinary care planning.

  • Facilitate educational groups and resource-focused sessions as assigned.

    Support discharge planning and continuity of care.

  • Maintain knowledge of community resources, recovery support services, and best practices in substance use disorder treatment.

  • Uphold all confidentiality requirements, including HIPAA and 42 CFR Part 2 regulations.

QualificationsPreferred
  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Criminal Justice, or a related field.

  • Master's degree preferred.

Required
  • Minimum of four (4) years of experience working in behavioral health, substance use disorder treatment, recovery support services, or case management.

  • Knowledge of substance use disorders, co-occurring disorders, trauma-informed care, harm reduction principles, and recovery-oriented systems of care.

  • Strong communication, organizational, and documentation skills.

  • Ability to establish professional relationships with clients from diverse backgrounds.

  • Experience utilizing electronic health record systems.

  • Valid driver's license and reliable transportation.

Preferred Certifications
  • CADC, CRADC, CAC, CPRS, CRPR, QMHP, or similar credential.

  • Experience working within ASAM levels of care.
