A Day in the Life

As an Aftercare Case Manager, you'll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You'll:

  • Focus on risk management and help families maintain stability during the aftercare period.
  • Provide crisis intervention and work with public and private agencies offering supportive services.
  • Maintain accurate documentation and reports.
  • Coordinate community and educational services for families.
  • Meet with families in-person to support reunification and resolve any concerns.
