Position Overview

Back on Track Counseling & Consulting is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team. Work Environment

This position is set within a detox center where we prioritize a supportive, remote work setting. This flexibility allows our team members to effectively manage their work-life balance while remaining engaged in meaningful patient interactions. The remote structure also provides you with the tools needed to facilitate telehealth sessions, ensuring continuity of care for patients who may not be able to attend in-person appointments.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patients’ needs, strengths, and areas for growth, ensuring each assessment is finely tailored to reflect the individual's situation.
  • Deliver high-quality counseling and health education interventions that address both psychological and physical health concerns, utilizing evidence-based practices to inform strategies.
  • Collaborate with patients and an interdisciplinary healthcare team to devise and execute personalized care plans focusing on behavior modification, healthy coping mechanisms, and significant lifestyle changes.
  • Engage actively with patients by setting collaborative goals, facilitating their empowerment to actively manage chronic conditions and mental health issues.
  • Regularly outreach to patients to support ongoing engagement and motivation, using a combination of synchronous and asynchronous communication methods.
  • Meticulously document patient interactions, treatment progress, and care plans using our electronic health record (EHR) system, ensuring compliance and quality in every patient record.
Patient Population / Client Focus

You will primarily engage with a diverse clientele, including individuals struggling with substance use disorders and various mental health conditions. Our patient population comes from varying backgrounds, and your role will be pivotal in fostering cultural competence and empathy, essential traits for connecting with patients from all walks of life. Understanding their unique experiences will help guide your interventions and improve their overall treatment outcomes.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A minimum of 3 years of clinical experience in a mental health or outpatient environment, ideally with a focus on substance use and addiction recovery.
  • Demonstrated proficiency in evidence-based therapeutic modalities, including cognitive behavioral therapy, motivational interviewing, and solution-focused therapies.
  • Exceptional communication, interpersonal, and assessment skills to effectively engage with patients and collaborate with a multidisciplinary team.
Preferred Experience
  • Experience in a detox or addiction treatment setting is highly favored.
  • Familiarity with electronic health record (EHR) documentation and telehealth platforms.
  • A deep understanding of the challenges faced by marginalized populations and the ability to tailor care accordingly.
