Viventium Home Care Agency is seeking a compassionate and skilled Registered Nurse to join our team in New York, New York. As a dedicated home care provider, we pride ourselves on delivering high-quality patient care and support to those in need. Responsibilities:

  • Provide direct patient care to individuals in their homes, focusing on promoting health, preventing illness, and alleviating suffering

  • Conduct initial and ongoing assessments of patients' physical, emotional, and social needs

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
