Job description

Under the direct supervision of the PACE Clinical Manager, the Geriatric Services Supervisor is responsible for coordinating, supervising and evaluating the implementation of psychosocial support services to participants of the Central Valley PACE program including participant screening, assessment, planning, coordination of patient services and care, counseling and referral services carried out by the Geriatric Services Support (GSS) team, and clinical supervision and training of the GSS team.

Schedule : Monday – Friday, working 8:00am – 5:00pm.

$104,332.80 - $114,766.08 Salary

Duties and Responsibilities

  • Provide on-site leadership and clinical support to the GSS team including general oversight of psychosocial services delivered to PACE participants and family members.
  • Develop and implement psychosocial service standards and procedures for the GSS team.
  • In partnership with PACE Center Manager, interviewing and training employees, planning assigning and directing work, appraising performance, recognizing and rewarding employees, supportive coaching, addressing concerns and resolving problems.
  • Conducts biopsychosocial assessments, participates in care planning and acts as a liaison between the participant and the interdisciplinary team.
  • Maintains accurate and current documentation in electronic medical record including written case management records, and periodic reassessments of program participants.
  • Provides the appropriate interventions and referrals to address the mental, emotional, social and physical health needs of participants and caregivers.
  • Facilitates communication among service providers, family, participants, and caregivers.
  • Coordinates 24-hour care delivery through collaboration with IDT members.
  • Provides individual and family counseling, develops and leads group counseling and activities.
  • Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants.
  • Facilitates, mediates, and documents participant care conferences, family meetings and facility partnership meetings.
  • Collaborates with local hospitals, SNF and LTC facilities for discharge planning and placement for appropriate participants in coordination with provider recommendations.
  • Monitors and evaluates significant changes in participant physical, cognitive and emotional status and communicate changes to IDT team.
  • Carries out leadership responsibilities in accordance with the organizations policies and procedures applicable to state and federal laws.
  • Model organizational culture to GSS department to ensure morale, teamwork, and that the positive employment culture of the organization is maintained.
  • Participates in leadership and other committees and meetings as requested by center manager.
  • Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.
  • Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
  • Provides information and education regarding PACE services, acting as a liaison between different disciplines, services and participants to ensure continuity of care and services.
  • Address any participants’ inquiries, providing guidance and assistance with any coordination and delivery of services.
  • Conducts home visits, hospital visits, nursing skill facilities visits as required.
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Provides weekly group and individual clinical supervision up to two units to unlicensed GSS.
  • Provide direct patient care to PACE patients with behavioral health problems or social service needs during scheduled office hours. This includes facilitating care through triage, collaboration, and referrals to other mental health providers when higher level of care or specialty services are needed.
  • Assist providers in recognizing and treating behavioral health and psychosocial problems. Assess the clinical status and/or conduct risk assessments of patients and provide appropriate level of intervention.
  • Other projects and duties as assigned.

Physical Demands

  • Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. 
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment

  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Moderate pressure to meet scheduled appointments while dealing with frail and confused participants.
  • Subject to unpleasant odors.
  • The noise level in the work environment is usually quiet to moderate.

Education/Experience Requirements

  • Minimum three (3) years health related experience.
  • Minimum one (1) year experience working with the frail or elderly population.
  • Two (2) years supervisory experience in a post licensure clinical capacity preferred.
  • Graduation from an accredited institution with a Master’s Degree in counseling, psychology, social work, or closely related field preferred.
  • A license from the State of California as a Licensed Clinical Social Worker (LCSW), or Clinical Psychologist (Ph.D).
  • Current BLS certification for Providers through the American Heart Association. Current BLS CPR Card certified by the American Heart Association.
