Position Summary
As a UAS Assessment Nurse (RN), you will play a vital role in evaluating the clinical, psychological, and social needs of members in the community. This field-based position involves conducting in-home and nursing home assessments using the UAS-NY system, ensuring compliance with New York State Department of Health regulations. You will contribute to developing person-centered care plans and supporting improved patient outcomes.
Responsibilities

  • Shift details: Monday - Friday (Business Hours)
  • Hours per week: 40
? Key Duties:
  • Conduct initial and reassessment visits, determining eligibility and care needs using UAS-NY
  • Perform functional, psychosocial, and environmental safety assessments
  • Develop and communicate individualized, person-centered service plans
  • Educate members and caregivers on program benefits, services, and guidelines
  • Submit accurate, timely documentation and communicate urgent needs to care teams
