Senior LTSS Service Care Manager (RN)

Bronx, NY (Field-Based โ€“ Must Reside in Bronx Area)
$36.21 โ€“ $65.09 per hour + up to 8% annual bonus

UAS Assessment Registered Nurse (RN) Job Summary

We are seeking an experienced Senior LTSS Service Care Manager (RN) to provide high-quality, person-centered care coordination for complex and high-acuity long-term services and support (LTSS) members in the Bronx area.

This is a field-based role requiring regular home and community visits, including completion of 2โ€“4 UAS assessments daily.

UAS Assessment Registered Nurse (RN) Key Responsibilities

  • Conduct in-home and field-based assessments and reassessments (UAS)
  • Develop, implement, and continuously update individualized care plans
  • Assess high-risk members and recommend appropriate care strategies
  • Coordinate services across providers, specialists, and community resources
  • Monitor member status, clinical changes, and progress toward care goals
  • Educate members, families, and caregivers on care plans and available benefits
  • Serve as a liaison between members, families, providers, and care teams
  • Identify care gaps and ensure compliance with state and federal guidelines
  • Document all care management activities accurately and timely
  • Support transitions to higher levels of care when needed
  • May precept and mentor new clinical hires as needed

UAS Assessment Registered Nurse (RN) Required Qualifications

  • Active New York State Registered Nurse (RN) license
  • Must reside in the Bronx, NY area
  • Minimum 4 years of relevant RN experience
  • Experience performing UAS assessments
  • Ability to conduct daily field visits (Mondayโ€“Friday)
  • Ability to complete 2โ€“4 UAS assessments or reassessments per day
  • Strong care coordination and clinical assessment skills
  • Valid driver's license and reliable transportation preferred (public transportation acceptable)
  • Strong communication and documentation skills

UAS Assessment Registered Nurse (RN) Preferred Qualifications

  • Bachelor's degree in Nursing (BSN preferred)
  • Experience in Managed Care Organization (MCO) or care management
  • Experience working with high-acuity or complex populations
  • Knowledge of community resources and social service coordination
  • Leadership or mentoring experience (precepting new nurses)
