Job description

This position is responsible for providing transportation services for Centra PACE/Ground Transport under the direction of the Transport Supervisor(s). Transportation will be provided to PACE participants and patients to & from Centra facilities. The Driver will be responsible for providing a safe and enriching experience for every individual from door to door. Driver will possess good communication skills. The Driver will preform pre & post vehicle safety inspections, follow scheduled transports, report & handle vehicle maintenance issues and communicate all participant/patient needs to immediate supervisor. 

Required License: Valid driver's license in the United States

Essential Duties and Responsibilities:

  • Providing exceptional door to door care to all PACE Participants & Centra Patients
  • Communicating all participant/patient changes & needs to direct supervisor
  • Performing pre & post vehicle safety inspections, assuring vehicle is in proper operational condition and reporting any maintenance issues to immediate supervisor
  • Maintain cleanliness of interior & exterior of Centra vehicle
  • Following scheduled transports for PACE participants or patients & other potential trips communicated by direct supervisor
  • Maintain all necessary records/documentation regarding participants/patients & vehicles
  • Communicating frequent changes to direct supervisor 
  • Performs other duties as assigned.

Other PACE Functions:

  • Entering safely into participant's homes on a routine basis
  • Participates as an extension of the Transport Supervisor as a member of the interdisciplinary team & participating in participant's care plans
  • Must meet standardized set of competencies for specific position established by Centra PACE and regulatory agencies 
  • Delivery of participant medications, medical supplies and other medical necessities
