Job description

At BHcare, we believe that every individual deserves support, dignity, and a safe path to independence. We’re seeking a compassionate and motivated Transitional Counselor to join our team and provide hands-on support, skill-building, and emotional encouragement to adults working toward greater self-sufficiency and community integration.

This position is Per Diem and there are various shifts available. 
What You’ll Do:
• Provide daily living assistance including hygiene, meal planning, medication protocols, cleaning, and budgeting
• Develop and implement individual recovery plans in coordination with clients, treatment teams, and natural supports
• Facilitate rehabilitation groups and provide interventions to help clients build life skills and maintain stability
• Monitor and document client progress using electronic health records (EHR) and communication logs
• Provide transportation and accompany clients to appointments as needed
• Ensure safety and well-being of clients during all shifts, including crisis response and overnight supervision
• Complete referrals and linkages to mental health, vocational, and community resources
• Use principles of Motivational Interviewing and support clients with co-occurring disorders using the IDDT model
• Maintain medication logs and ensure safe protocols for self-medication
• Perform routine cleaning and third-shift documentation as required
Shift Flexibility:
Day, evening, and overnight positions available. Overnight staff are responsible for maintaining a safe, clean environment and monitoring clients while awake and alert throughout the shift.
What You’ll Bring:
• Education/Experience: Two years of college OR high school diploma/equivalent with 2 years of direct care experience
• Skills: Strong communication, basic computer proficiency (Microsoft 365), EHR navigation
• Mindset: Empathy, patience, organization, and a passion for helping others thrive
• Licenses: Valid driver’s license with insurable driving record
• Bonus: Public Passenger License (or willingness to obtain – not required for overnight shifts)

QUALIFICATIONS:

  • Two years of college education (human services or behavioral science preferred) or High School Diploma or Equivalent and 2-4 years direct care direct care work experience.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

LICENSURES/CERTIFICATIONS:

  • Ability to obtain and maintain CPR/AED/First Aid training
