Job description

Are you passionate about helping others build independence and thrive in their community? BHcare is seeking a Transitional Counselor to provide guidance, support, and skill-building for individuals in our Transitional Supervised Housing Program. This role is essential in empowering residents to reach their personal goals, develop life skills, and enhance their overall quality of life.
Schedule (40 Hours/Week):
• Saturday & Sunday: 8:00 am - 4:00 pm
• Monday, Tuesday & Wednesday: 4:00 pm - 12:00 am
What You'll Do:
• Assist in the daily operations of the program while ensuring a safe and supportive environment.
• Help clients create and follow individualized recovery plans, reviewing and updating as needed.
• Support residents in developing independent living skills such as hygiene, cooking, cleaning, budgeting, and health management.
• Provide counseling, emotional support, and rehab interventions, including facilitating groups.
• Assist clients with scheduling and attending personal appointments; provide transportation as needed.
• Collaborate with treatment teams, natural supports, and community resources to ensure comprehensive care.
• Conduct crisis assessments and interventions, coordinating with clinical or emergency services when necessary.
• Maintain accurate documentation of client progress, services, and recovery goals.
• Enter daily service activities into the database and ensure data accuracy.
• Stay awake, alert, and responsive during all shifts, prioritizing resident safety and well-being.

  • Two years of college education or High School Equivalent and 2 years direct care work experience
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver's license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan

Professional Licensure/Certifications:
  • Ability to obtain and maintain BLS certification
