JOB TITLE:Transition Coordinator

REPORTS TO:Director of School Program

HOURS/STATUS:Full-Time / Exempt

START DATE:ASAP

PAY:$2,070/pay period

JOB SUMMARY

The Transition Coordinator will provide vocational/educational opportunities in a work environment for students with disabilities by developing and maintaining a skills-based vocational program.

SPECIFIC JOB RESPONSIBILITIES
  • Assists the job developer with new internships worksites across potential employment opportunities.
  • Assists in the development, review, and update appropriate work plans for each assigned individual.
  • Facilitates communication with employers as appropriate which may include schedule adjustments, changes, and feedback of individual performance.
  • Assists in the development of a skillbased inschool work experience program.
  • Serves as a link between the agency and community work site setting.
  • Works closely with the school’s Transition teachers regarding student postsecondary transition goals.
  • Communicates with onsite employers about job training sites, including problems and concerns.
  • Collaborates with classroom teams to maximize student vocational experiences and skill development.
  • Communicates with parents regarding employability profile 2x per year.
  • Collaborates with the ETP program for paid student work experiences.
  • Maintains confidentiality and follow Agency policies for protecting the health, safety, and well being of individuals.
  • Attends Program staff and inservice meetings as required.
  • Attends professional development workshops as required by supervisor.
  • Completes any necessary paperwork as required by NYSED regulations.
  • Must be sensitive to the cultural diversity of others and facilitate trusting relationships and partnerships with individuals, families, community members, and coworkers.
  • Supervises assigned job coaches.
  • Provides onthejob training to students on the work site.
  • Identifies supports and adaptations as needed.
  • Other duties may be assigned.
  • QUALIFICATIONS

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Candidates for this position will have: the ability to demonstrate and provide examples of continuous personal development and improvement; person centered approach; excellent communication skills (verbal and written); Problem solving skills, strong organizational skills and the ability to effectively manage multiple tasks/projects. Must be able to lift, push, and pull 50lbs. Must maintain certification in Strategies for Crisis Intervention and Prevention.

    EDUCATION and/or EXPERIENCE

    Minimum of a bachelor’s degree in a Human Service field and Work-Based Learning Coordinator certification required. Two years of previous experience working with individuals with developmental disabilities in a supported work environment.
