Pay: $34.97 โ€“ $43.71 per hour
Job Type: Temporary
Schedule: Monday to Friday, 9:00 AM โ€“ 5:00 PM


Position Summary

The Osborn is seeking a compassionate and dedicated Temporary Social Worker to provide high-quality social work services to residents of the Osborn Pavilion. The primary goal of this role is to support residents in managing the challenges of aging, illness, and long-term care living while promoting the highest possible quality of life.


Key Responsibilities
  • Assess and address residentsโ€™ psychosocial and emotional needs
  • Conduct admission psychosocial assessments
  • Educate residents and families on advance directives and resident rights
  • Provide counseling, support, and case management services
  • Document care plans, interventions, and progress notes
  • Participate in interdisciplinary care planning meetings
  • Complete MDS 3.0 assessments in a timely manner
  • Collaborate with families, caregivers, and community resources
  • Coordinate discharge planning and transitions of care
  • Facilitate referrals to home care and outpatient services
  • Communicate with insurance providers regarding coverage
  • Support family meetings and support groups
  • Provide education to staff and families
  • Serve as designee for Director of Social Services as needed

Qualifications
  • Bachelorโ€™s Degree in Social Work (BSW) required; MSW preferred
  • Knowledge of Alzheimerโ€™s disease, dementia, and aging populations
  • Strong communication and interpersonal skills
  • Ability to work in a collaborative, team-based environment
  • Compassionate, patient, and professional demeanor
  • Strong organizational and documentation skills
  • Computer proficiency, including EMR systems

Work Environment & Physical Requirements
  • Ability to move throughout the facility and interact with residents regularly
  • Ability to sit, stand, and bend for extended periods
  • Occasional lifting (up to 50 lbs)
  • Ability to manage the emotional demands of the role
