Are you a compassionate and experienced Nurse Practitioner looking for a flexible role that allows you to make a direct impact in patients' lives? We're seeking a dedicated Family Nurse Practitioner (NP) to provide in-home assessments and care planning services throughout the NYC.

Assignment Details:
  • Position: Family Nurse Practitioner (NP)
  • Location: NYC (Hybrid Field-Based Role)
  • Schedule: Flexible, Minimum 9 Hours Weekly
  • Initial Patient Visits: Approximately 2.5 Hours Each
Requirements:
  • Active New York Nurse Practitioner License
  • Active NY Medicaid Provider enrollment
  • Must have NYS Driver's license or NYS Id
  • ANCC certification
  • NPI number and malpractice insurance
  • Minimum 2 years (or 3,600 hours) of NP experience in:
    • Family Health
    • Adult Health
    • Acute Care
