Job Description

Are you an experienced RN Telephonic Care Manager looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered "yes" to any of these questions - this is the position for you!


Daily Responsibilities:  

You, as the RN Case Manager will be held responsible to act as the Health Coach providing telephonic outreach support to eligible participants to promote health and wellness programs and assist members in receiving preventive services to achieve health management and self-management goals.  

You'll have the responsibility of working towards improving overall quality metrics ratings by bringing members into compliance and closing gaps in care.


Qualifications

Requirements: 

3+ years clinical experience required [any healthcare setting]

RN with active, clear Iowa license

Case Management experience

Hours for this Position:

Monday - Friday 8:00am to 5:00p [May work some weekends] 

If Contract, Length of Assignment: 10-11 Months

Compensation Notes:

Competitive Salary: $29 to $31.25 an hour [$65K]
