Job description

Salary: $43,000 - $45,000 DOE

REPORTS TO: Home Based Services Supervisor


STATUS: Exempt


ABOUT THE ROLE:

The Support Services Provider delivers community-based support services to individuals affected by mental health disorders, substance use disorders, or co-occurring conditions. This position is responsible for providing direct, face-to-face services that help clients develop the skills necessary to live independently and successfully within their communities. Services are provided in clients homes, community settings, and other supportive environments as appropriate to meet individual needs.


RESPONSIBILITES:

  • Provide individual and group support services in clients homes, at Heartland Counseling Services (HCS) facilities, during transportation, and in other appropriate community settings.
  • Maintain a service schedule that ensures accessibility and responsiveness to client needs, including evening and weekend availability as required.
  • Meet established monthly productivity and billable service expectations.
  • Provide support, screening, and crisis intervention services during emergencies, including after-hours and weekend coverage when necessary.
  • Participate in the agencys on-call rotation to ensure 24/7 crisis response coverage.
  • Deliver services at other HCS locations as assigned, which may require travel.
  • Teach and reinforce skill-building strategies that support recovery, independence, and wellness related to mental health and/or substance use challenges.
  • Collaborate and communicate effectively with clients, family members, community partners, and other service providers.
  • Provide community education and outreach presentations to schools, law enforcement agencies, community organizations, media outlets, and other groups as assigned.
  • Demonstrate flexibility, adaptability, and a commitment to professional growth through ongoing training, supervision, and case consultation.
  • Maintain accurate, timely, and complete client documentation, including assessments, treatment plans, progress notes, and discharge summaries.
  • Attend and actively participate in supervision sessions, staff meetings, community-based meetings, and other required agency meetings.
  • Participate in client observation sessions with supervisors as scheduled.
  • Complete all required documentation in compliance with agency policies, procedures, and regulatory standards.
  • Administer screening and assessment tools as directed.
  • Participate in agency-sponsored community events and outreach activities as assigned.
  • Serve on agency committees as assigned.
  • Perform other duties and responsibilities as assigned.


REQUIRED SKILLS AND ABILITIES:

  • Ability to establish and maintain positive, cooperative, and supportive relationships with clients, colleagues, and community partners.
  • Commitment to professional ethics, client confidentiality, and compliance with applicable regulations and standards.
  • Ability to maintain required certifications, licenses, and training credentials.
  • Knowledge of recovery-oriented skill-building practices, behavioral health services, and community resources.
  • Strong interpersonal skills and the ability to work effectively with individuals from diverse backgrounds and experiences.
  • Excellent verbal and written communication skills.
  • Strong organizational, time management, and documentation skills.
  • Ability to work independently while maintaining effective collaboration with a multidisciplinary team.
  • Possession of a valid drivers license and an insurable driving record.


EDUCATION & EXPERIENCE:

  • Bachelors degree in the human services field preferred or two years experience in a related field.
  • State of Nebraska Peer Support Specialist Certification in good standing.


PHYSICAL REQUIREMENTS:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
