Job description

S:US Is seeking an Aftercare Specialist for their short term homeless prevention program.  

This position  is located at West 181st Street (St. Nicholas Ave) NY, New York 10033

 

The Aftercare Specialist will provide aftercare services to  program participants and their families.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Create a resource bank that will serve all HomeBase clients
  • Meet with clients who may need more intense services to tap into resources to adjust to community
  • Attend appointments (other than court) with clients who need additional instructions
  • Complete home visits for Aftercare clients
  • Accompany Outreach Specialist to presentations in order to acquire additional resources
  • Assists families with identifying and establishing relationships with supports
  • Act as a liaison between families, case managers and other providers
  • Provide needed services through referrals and follow up on progress
  • Assist Housing Specialist with establishing and maintaining relationships with landlords and brokers who are willing to work with HomeBase clients
  • Maintain tracking system for renewals of City rental subsidy
Qualifications

REQUIRED EDUCATION AND EXPERIENCE

  • High School Diploma or GED is with 3+ years' experience required.
  • Experience working with substance abuse, homelessness, families and groups.
  • Excellent written and oral communication skills
  • Basic computer skills required
