Relocation is provided for the right Candidate.Job Overview:Join our healthcare team as a Board-Certified or Board-Eligible Internal Medicine or Family Practice Physician. In this role, you will lead patient care in a collaborative environment, managing complex cases with the support of advanced practice providers, social workers, case managers, and other health professionals. You'll participate in multidisciplinary conferences to focus on high-risk patients. Our facility provides access to advanced technology, including a robust EMR system and a proprietary decision-support tool that consolidates data from various sources into actionable insights to enhance patient care.Responsibilities:- Deliver evidence-based primary care, with a focus on senior patients.- Conduct annual wellness exams for new and existing patients.- Perform minor office procedures as needed.- Coordinate patient care with specialists and ancillary providers.- Ensure patients have advanced care planning, and educate them on health topics such as disease management, medications, healthy lifestyle choices, and end-of-life care.- Participate in and sometimes lead interdisciplinary team conferences.- Manage population health for your assigned patient panel.- Work towards achieving HEDIS and CAHPS goals.- Strive to provide exceptional patient experiences and accessibility.- Uphold a commitment to innovation and the continuous improvement of healthcare practices.- Contribute to quality improvement, peer review, and special projects as required.- Adhere to HIPAA regulations and maintain the confidentiality of protected health information (PHI).Supervisory Responsibilities:- Assist in supervising medical assistants and other office staff.- May directly supervise advanced practice clinicians.Required Skills and Experience:- Preferably 2 years of experience as an Internal Medicine or Family Practice clinician.- 1-3 years of managed care experience is preferred.- Basic understanding of utilization management and quality management processes.- Completion of a U.S. residency program in Internal Medicine or Family Medicine.- Board certification or the ability to achieve board certification within 6 months of hire.- Possession of a medical license to practice in the state of employment.- Substantial knowledge of various medical subspecialties.- Clinical confidence and an approachable "bedside manner."- Flexibility and a willingness to learn new systems and processes.- Strong communication skills.
