Bright Path Social Work is seeking a dedicated and compassionate Senior Licensed Social Worker to join our remote team, specializing in geriatric care. This position is a remote role, allowing the Senior Social Worker the flexibility to manage their schedule while maintaining a strong connection with clients. The work hours typically span from Monday to Friday, 8:00 AM to 4:30 PM, with some evening or weekend availability required to address client needs. Our team prioritizes a collaborative and supportive environment, leveraging technology and regular communication to ensure continuity of care.

• Conduct thorough assessments of seniors' psychosocial needs, accounting for emotional, mental, and social factors to develop holistic care plans.
• Collaborate closely with clients and their families to establish personalized care plans that align with individual goals and preferences.
• Provide emotional support and counseling, helping seniors navigate challenging life transitions, including health changes and loss.
• Advocate for client rights, assisting families in accessing vital community resources and services catered to seniors.
• Work alongside an interdisciplinary team of healthcare professionals—including nurses and caregivers—to synchronize care efforts and improve outcomes.
• Maintain consistent communication with clients through regular home visits to monitor their well-being, assess living conditions, and promptly address any concerns.
• Educate clients and their families on available resources, promoting social engagement and active participation in community life.
• Document all client interactions, assessments, and care plans in our electronic health record (EHR) system meticulously to ensure compliance with industry standards.
• Stay updated on current social work practices and policies to guarantee that our services meet ethical and regulatory standards.

The primary focus of this role is on the senior population, which includes individuals aged 65 and older facing various challenges related to aging. You will work closely with clients experiencing physical and cognitive health decline, social isolation, and the emotional ramifications of aging. Your efforts will directly contribute to improving the quality of life for these individuals and their families.

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.
• A minimum of two years of experience in social work, with a preference for backgrounds in community centers or geriatric settings.
• Solid understanding of social work principles and aging-related issues, coupled with knowledge of community resources.
• Exceptional skills in assessment, counseling, problem-solving, and communication.
• Empathy, compassion, and the ability to build rapport with seniors and their families.
• Proficient in using electronic health record systems and other necessary computer applications.

• Experience utilizing evidence-based practices and methodologies specific to geriatric care.
• Familiarity with community health services and support systems available for seniors.
• Background in managing multidisciplinary teams and contributing to coordinated care efforts.
