Position Summary:

The Social Worker provides comprehensive psychosocial assessments, care coordination, counseling, crisis intervention, and case management services to individuals receiving services through After Hours Project. The Social Worker collaborates with healthcare providers, behavioral health professionals, housing specialists, and community partners to ensure clients receive coordinated, high-quality care that addresses both medical and psychosocial needs.

Essential Duties and Responsibilities:

Clinical Services:

• Conduct comprehensive biopsychosocial assessments. 
• Develop individualized service plans in collaboration with clients. 
• Provide supportive counseling, crisis intervention, and short-term therapeutic interventions. 
• Assess clients for mental health, substance use, housing, financial, and social service needs. 
• Identify barriers to treatment engagement and develop strategies to improve participation in care. 
• Utilize evidence-based approaches including Motivational Interviewing, Trauma-Informed Care, Harm Reduction, and Strengths-Based Practice. 

Case Management & Care Coordination:

• Coordinate medical, behavioral health, housing, and supportive services. 
• Link clients to primary care, specialty medical care, mental health providers, substance use treatment, housing resources, transportation, food assistance, and entitlement programs. 
• Monitor client progress toward established goals. 
• Facilitate referrals and ensure successful linkage to community resources. 
• Participate in interdisciplinary case conferences. 

Behavioral Health Support:

• Screen clients for depression, anxiety, trauma, substance use disorders, and suicide risk. 
• Provide psychoeducation regarding mental health and wellness. 
• Promote engagement in behavioral health treatment. 
• Assist clients in overcoming stigma associated with mental illness and HIV. 

Advocacy:

• Advocate for clients within healthcare, legal, housing, and social service systems. 
• Assist clients with insurance enrollment and benefit applications when appropriate. 
• Educate clients regarding available resources and their rights. 
• Empower clients to actively participate in healthcare decision-making. 

Documentation & Compliance:

• Maintain accurate, timely, and complete client records in Electronic Medical Records (EMR). 
• Complete required assessments, progress notes, service plans, and discharge summaries. 
• Ensure compliance with HIPAA, OMH, DOHMH, Ryan White, and agency policies. 
• Participate in quality improvement initiatives and program audits. 

Community Engagement:

• Participate in outreach activities and community education events. 
• Build collaborative relationships with healthcare providers and community organizations. 
• Assist with retention and re-engagement efforts for clients who have fallen out of care. 

Minimum Qualifications:

• Master's Degree in Social Work (MSW) from an accredited institution, required.
• Licensed Master Social Worker (LMSW) preferred, but not required.
• Minimum of two years of experience providing case management or behavioral health services. 
• Experience working with HIV/AIDS, LGBTQ+ populations, behavioral health, or underserved communities preferred. 
• Knowledge of trauma-informed care and culturally responsive practice. 
• Excellent interpersonal, communication, and documentation skills. 
• Proficient in Microsoft software

Preferred Qualifications:

• Experience with Ryan White-funded programs. 
• Knowledge of HIV prevention, treatment adherence, and viral suppression initiatives. 
• Familiarity with Medicaid Managed Care and New York State healthcare systems. 
• Bilingual (English/Spanish) strongly preferred. 
• Experience providing crisis intervention. 
• Training in Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
