Position Overview:

We are seeking a dedicated and empathetic Social Worker to join our team. The Social Worker will play a vital role in assessing residents' psychosocial needs, providing counseling and support services, and coordinating community resources to enhance their well-being.

Social Worker Key Responsibilities:

  • Conduct psychosocial assessments of residents to identify needs, preferences, and goals.
  • Develop and implement individualized care plans in collaboration with residents, families, and interdisciplinary team members.
