Job description

Social Worker

The primary role of the facility Social Worker is to coordinate, supervise and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility, and in accordance with applicable state and federal regulations and professional standards.

Responsibilities:

  • Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors.
  • Participate in planning for resident discharges.
  • Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.
  • Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed.
  • As needed, participate in the clinical evaluation and screening of potential admissions to the facility.
  • Work in collaboration with the facility interdisciplinary treatment team in accessing information regarding, and as appropriate making referral to, community based services for residents who wish to access such services.
  • Serve as a facility treatment team liaison with facility contracted mental health practitioners and consultants.
  • Actively participates in developing and implementing new behavioral and psycho social service programs and the presentation of such programs to physicians, family members, and residents.

Qualifications:

Bachelor’s Degree in Social work or related human services field, required.

A minimum of one years’ experience in long term care providing direct social services, preferred.
