Make a Meaningful Impact in Long-Term Care

Brownsburg Health Care Center is seeking an experienced Social Services Director to lead psychosocial services in our Skilled Nursing Facility (SNF). This role is ideal for a licensed social worker passionate about resident advocacy, care planning, discharge planning, and interdisciplinary collaboration in a long-term care setting.

Key Responsibilities
  • Complete comprehensive psychosocial assessments for residents

  • Develop and implement individualized care plans

  • Provide emotional, social, and mental health support to residents and families

  • Lead discharge planning, transitions of care, and community resource referrals

  • Advocate for resident rights, dignity, and quality of life

  • Collaborate with nursing, therapy, and interdisciplinary care teams

  • Maintain accurate documentation in compliance with state and federal regulations

  • Participate in state surveys, audits, and compliance reviews

  • Supervise or support social services staff, as applicable


Required Qualifications
  • Bachelor’s degree in Social Work or related Human Services field (Psychology, Sociology, Human Services, Rehabilitation Counseling)

  • Licensed, certified, or registered to practice in Indiana

  • Minimum 1 year of supervised social work experience in a healthcare setting

  • Knowledge of CMS regulations, resident rights, and discharge planning processes

Preferred Qualifications
  • 2+ years of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) environment

  • Previous Social Services Director or leadership experience
