) Salem Health & Rehabilitation Center in Salem, Virginia is seeking a Full-Time Discharge Planning Assistant. As a member of our team, you will enjoy a genuine spirt of caring that makes our center so well respected in our local community. Become part of an enthusiastic and dedicated team of professionals who share their positive attitudes and compassionate hearts with every patient, family, and co-worker.
Salem Health and Rehab provides a high volume of short-term nursing and rehabilitation services. We are seeking a candidate that can provide discharge planning as well as comprehensive medically related social services to our patients. Responsibilities include MDS, Care Plans, Assessments and timely documentation operating within the framework for the company's standards for best practice in accordance with all state and federal regulations.
The qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or other human services related degree.
We offer a competitive rate of pay and a comprehensive benefits package for full time associates which include affordable health and dental insurance within 60-90 days of hire, paid time off, extra pay for holidays, and a 401k with company match.
