We are looking for a Social Worker who is compassionate about working with older adults in our Adult Day Health Care Center.
QUALIFICATIONS:
  • Mater's degree in social work or in psychology, gerontology, or counseling plus one year of experience in aging, health, or long-term care. 
  •  A Bachelor's degree in Social Work (BSW) plus two years of experience in the fields of aging, health, or long-term care. 
  • Vietnamese, Cantonese or Mandarin preferred
Duties/Responsibilities:
  • The Social Worker will screen all referrals to determine the appropriateness of the prospective participants for the full assessment process and, Unicare ADHC participants
  • Provide referral and follow‑up for persons not appropriate for adult day health care services.
  • Conduct an initial assessment and reassessment of each participant's mental, social and emotional status; and ability of the participant and family to cope with the disability.
  • Develop an individualized plan of care, if indicated by the assessment, including long and short‑term goals, and participation of the family and other individuals, if indicated.
  • Provide counseling, group or individual, to the participant's family, if indicated.
  • Provide problem‑oriented discussion groups for participants (and family members as called for).
  • Act as a member of the multidisciplinary team and participate in case conferences.
  • Provide referral to and assistance, if indicated, in obtaining other needed community services.
  • Serve as a liaison with the participant's family and home.
  • Serve as a liaison with other community agencies that may be providing services to a participant and work with these agencies to coordinate services and avoid duplication.
  • Provide discharge planning; be responsible for coordinate and implementation of the plan.
  • Record, sign and date Flowsheet and Progress Notes in the participant's chart in a timely manner.
  • Provide supervision for social work assistants, student interns or aides and define their responsibilities.
