Ambercare is currently seeking a Full Time Service Coordinator out of their Albuquerque NM location! Responsible for scheduling and supervising in-home care workers and clients in a geographic area. If you seek a challenging position with the satisfaction of knowing that you have helped older people and people with disabilities live safely at home, this is the job for you! Also will be responsible for conducting in-home visits at clients' homes to monitor and report on quality of services being provided by Home Care Aides, and to determine whether existing care plans are adequate to meet client needs.

Location: Ambercare 2129 Osuna Rd. NE Albuquerque, NM 87113

Schedule: Monday thru Friday 8:00AM to 5:00PM Some occasional Evenings & Weekends per business needs

Essential Duties:

  • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
  • Provides alternate coverage to ensure the clients care plan is followed and client services are not interrupted.
  • Contacts care providers and clients to provide service updates
  • Monitors clients condition and performance of duties by Home Care Aides
  • Maintains all company and Department on Aging rules, regulations, and standards
  • Conducts home visits to new clients to welcome and review the Welcome Packet
  • Observes and evaluates Home Care Aides in the client's home as directed by Supervisor, to assure client Care Plans are being followed
  • Prepares and completes accurate evaluation reports and case notes as needed
  • Maintains schedules to assure timely completion of all assigned home visits
  • Attends weekly branch meetings
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned

Position Requirements & Competencies:

  • Minimum of 2 years experience working in the field of senior services or a related social service field.
  • Excellent oral and written communication skills.
  • Strong interpersonal skills.
  • Ability to prioritize and handle high call volumes.
  • Client-focused and customer service-driven.
  • Proficient in Microsoft Word, Excel, and Outlook.
  • Must have reliable transportation, valid drivers license and insurance for travel within the branch service area
