Job description

Description

Compass Health, the largest provider of skilled nursing care in SLO County, is looking for a LVN or RN with MDS experience to join our team. The ideal candidate will be flexible to cover multiple facilities in SLO County.


Responsibilities of the MDS Nurse:

  • Oversee utilization of interdisciplinary rehab team process in formulating MDS/RAI's.
  • Observe direct nursing care, review documentation,  and report all discrepancies noted concerning physician's orders, diet change, charting errors, etc., to the Nurse Supervisor 
  • Plan, schedule and revise the MDS, including the implementation of RAPs, triggers, and care plans 
  • Complete the entire MDS process in a timely and consistent manner.
  • Ensure the successful, timely electric transmission of the MDS package.

Must have excellent leadership and communication skills;

Must be a licensed LVN/RN in the state of California;

Must have skilled nursing experience;

Must have basic MDS experience.
