E3 Case Managers can be a steppingstone to leadership or advanced roles (e.g., hybrid supervising nurse). Other opportunities include certification support (e.g. CCM) and paid training opportunities.

General Job Description:

E3 Case Managers is currently looking for a full-time case manager willing to travel throughout the metropolitan Detroit, Michigan region to work with our clientele. The CM position is responsible for developing comprehensive rehabilitation plans and initiating case management efforts to maximize patient recovery so they can reach their greatest level of independence.

Major Duties and Responsibilities:

-Communicate regularly with care team providers (i.e. physicians, PT’s, OT’s, nursing staff, etc.) and their families about Plans of Care and other treatment protocols

-Answer patient questions about their care, treatment plans, illness progression, etc. so they feel safe and secure in our care

-Follow E3 processes, procedures, and protocols so clients receive consistent care

-Monitor and adjust patient’s status based on changing needs and conditions

-Address any patient concerns with appropriate professional decisions

-Oversee all intake and discharge activities

Position Qualification(s):

-Licensed RN nurse in the state of Michigan

-Willing to obtain a CCM certification after the first year of employment

-Valid driver’s license

-Valid auto insurance coverage

-Reliable transportation

-Must be willing to drive up to 1.5 hours to see patients

-Attend both virtual/remote or in-person appointments

- Submission of timely, daily client dictation and/or reporting

-Supervised by Director of Nursing

-1-2 years in case management preferred

-Strong written and verbal communication skills

-Familiarity with Windows-based operating systems and EMR programs
