Guided by the CMSA standards, we focus on advocacy, communication, and coordination to ensure every client receives safe, effective, and compassionate care.

Role Overview
We are seeking a compassionate, detail-oriented Field Case Manager to support patients and collaborate with a multidisciplinary clinical team. This role involves direct patient interaction, care coordination, advocacy, and field-based travel.
Key Responsibilities
  • Coordinate and manage patient care plans in collaboration with the clinical team
  • Advocate for patients and monitor health status, reporting new or changing conditions
  • Maintain accurate, timely documentation and case notes
  • Communicate effectively with patients, families, providers, and internal stakeholders
  • Manage multiple cases while maintaining organization and attention to detail
  • Travel to client locations (up to 80 miles as needed)
  • Work effectively under pressure in dynamic, real-world environments

Minimum Qualifications (Must-Haves)
  • Bachelor's degree (any field) OR:
    • Licensed Practical Nurse (LPN) or Registered Nurse (RN) accepted in lieu of degree
  • Strong communication, organization, and teamwork skills
  • Demonstrated integrity, accountability, and reliability
  • Compassionate and patient-focused mindset with strong advocacy skills
  • Ability to manage responsibilities in a fast-paced, high-pressure environment
  • Valid driver's license, reliable vehicle, and active auto insurance
  • Willingness and ability to travel regularly for patient visits (up to ~80 miles)
  • Authorized to work in the U.S.

Preferred Qualifications (Nice-to-Haves)
  • Degree in healthcare, social services, or related field
  • 1-2+ years of experience in healthcare or case management
  • Experience working with:
    • Traumatic Brain Injury (TBI)
    • Spinal cord injury patients
  • Case management or patient-facing experience (hospital, clinic, home care, or similar)
  • Bilingual or multilingual abilities
