What You'll Do:

As a Case Manager for our Respite Program, you'll help connect children, caregivers, and respite providers with the services and support they need to thrive. This is more than a case management job - it's an opportunity to reduce caregiver stress, strengthen families, and create meaningful moments of relieve and connection. Respite services provide temporary relief for youth and primary caregivers of youth with disabilities, chronic illnesses, or other special needs.In this role, you'll serve as a trusted partner for individuals and families by:

  • Conducting assessments and determining eligibility for respite services
  • Developing and monitoring individualized service plans
  • Coordinating services and connecting families with community resources
  • Supporting caregivers by identifying needs and advocating for appropriate services
  • Building supportive relationships with caregivers, children and respite providers
  • Maintaining accurate documentation and case records
  • Monitoring program compliance and service quality
