• Conduct individual and group therapy sessions via telehealth platforms to support clients dealing with various mental health issues.

• Implement crisis interventions as necessary, providing immediate support and resources to clients in distress.
• Collaborate with multidisciplinary teams, including medical professionals and family members, to coordinate comprehensive care and improve treatment outcomes.
• Utilize electronic health record (EHR) systems for accurate documentation of client interactions, treatment plans, and progress notes, ensuring compliance with all regulatory requirements.

Your primary focus will be on clients in hospice care, many of whom face complex psychosocial challenges associated with chronic illness and end-of-life considerations. This role demands a sensitive and nuanced understanding of grief, loss, and the emotional needs of clients and their families during difficult times. You will play a crucial role in providing comfort and support as patients transition through significant life moments.

• A Master’s degree in Social Work (MSW) or a related discipline.
• A valid and active Licensed Clinical Social Worker (LCSW) license in the state of California.
• A minimum of two years of clinical experience in a mental health setting, preferably with hospice or palliative care.
• Profound knowledge of mental health assessments, therapeutic interventions, and crisis management.
• Proficiency in utilizing electronic health records and other relevant software for documentation.

• Experience working within hospice or palliative care settings.
• Familiarity with telehealth platforms and virtual care practices.
• Demonstrated ability to work collaboratively in multidisciplinary teams.
