• Collaborate with physicians and other healthcare professionals to relay patient progress, communicate treatment needs, and address any concerns regarding patient care.
• Ensure thorough and timely documentation of clinical interactions, maintaining accurate clinical and progress notes in accordance with departmental policies.
• Participate in treatment plan development, assessing psychosocial factors to create effective treatment and discharge plans for patients.
• Provide direct counseling services, leveraging crisis intervention techniques to address emotional and behavioral challenges faced by patients.
• Assist clients in developing discharge plans and facilitate care coordination by connecting them with relevant community resources and services.
• Engage with families and support systems to foster comprehensive treatment strategies and ensure seamless transition of care.

This role involves working with a diverse array of patients, including individuals experiencing anxiety, depression, trauma, and other mental health disorders. Understanding the unique needs of our client population is crucial, and you will be responsible for providing culturally competent care that respects individual backgrounds and experiences.

• A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required.
• Possession of an active and relevant state licensure (LMFT, LPC, LCSW, or equivalent).
• A minimum of three years of experience in an outpatient mental health setting is essential.
• Demonstrated experience in utilizing evidence-based treatment modalities and mental health interventions.
• Strong self-direction abilities, with an emphasis on organization and time management.
• Proficiency in Microsoft Office applications and other relevant software tools to facilitate remote documentation and communication.

• Prior experience working in a community health clinic or a similar environment that focuses on patient-centered care.
• Familiarity with electronic health record (EHR) systems and telehealth platforms that enhance service delivery.
