Role Overview:
PharmD Live seeks a proactive Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our remote care team. This position supports patients enrolled in chronic care management (CCM) and remote patient monitoring (RPM) programs. The ideal candidate will provide high-quality patient care, education, and coordination, ensuring adherence to care plans while collaborating with multidisciplinary teams.
Key Responsibilities:

  • Conduct virtual outreach with patients to review care plans and provide education
  • Monitor patient health status via RPM tools and escalate clinical concerns appropriately
  • Document all interactions in compliance with CMS, payer, and internal quality standards
  • Coordinate post-discharge follow-up and specialist appointments
  • Collaborate with nurses, pharmacists, and other care team members to optimize outcomes
  • Foster patient engagement, adherence, and trust through empathetic communication
Required Credentials:
