Job description

Part time - 24 hours weekly

Days - 7am-7pm with every 6th weekend required

General Summary

Works collaboratively with the Capacity Coordinator (RN) to coordinate, communicate and predict impacts to bed management, transfers and admissions from all possible points of entry and provide general oversight of patient placement and patient flow throughout the Hospital/Health System

Qualifications
Minimum Education:

  • Certificate Program Post High School Vocational/Specialized Training. Required

Work Experience:

  • 1 year of LPN experience in an inpatient setting or 2 years of LPN experience required 

Licenses:

  • Licensed Practical Nurse Upon Hire Required or
  • Licensed Practical Nurse Multi State License Upon Hire Required

Courses and Training:

  • Completion of accredited LPN program. Upon Hire Required

Knowledge, Skills, and Abilities:

  • Typing and moderate computer skills and knowledge preferred.

Duties and Responsibilities
Essential Functions:

  • Under the guidance of the Capacity Coordinator (RN):
  • Assists with patient flow coordination and all associated activities with physicians, clinicians and healthcare professionals, to ensure timely patient placement based on the patient's care needs, diagnosis and acuity.
  • Promotes cooperation and effectively communicates to appropriate professionals throughout the WellSpan Health System to achieve the objectives of the WellTrack Patient Placement and Transfer Center regarding the improvement of capacity and patient flow for bed placement and transfer.
  • Coordinates and facilitates direct admissions and transfers, including making appropriate transportation arrangements and coordination of care.
  • Uses clinical judgment, based on acquired nursing skills, scope of practice, knowledge base and education to assess patients' care delivery needs, acuity, diagnosis and required specialty services in relation to patient placement and available clinical resources.

  • Coordinates and integrates services within the department/procedural areas/entity, with other departments/procedural areas/entities, and with WellSpan Health System's primary functions.
  • Continuously improves the department/entity/system's performance using prescribed system performance improvement strategies.
  • Demonstrates a commitment to patient, visitors and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  • Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
