Job description

Prestige Staffing is searching for a remote Care Navigators for a contract to hire role. Applicants must live in the lower 48 states yet cannot live in CA. The ideal candidate will have a dedicated quiet work space in their home and high speed internet. We are searching for candidates who have worked in a medical call center environment who know medical terminology and have a passion for helping others. We need a compassionate individual who is not afraid to ask tough questions and seek answers in order to help get the patient a better quality of life.
Details:
Pay: $16-21/hour (based on experience)
Location: 100% Remote
Hours: M-F shifts will range between 7a-9p depending on time zone
Summary:
The Care Navigator plays a vital role in assessing member needs, advocating for essential services, and providing ongoing support to help individuals thrive within their communities. This remote position involves conducting intake interviews, connecting members with healthcare and social resources, and tracking progress to ensure effective care coordination. The role emphasizes community engagement, health education, and data reporting to support program outcomes.
Requirements:

  • Experience in case management, healthcare advocacy, or social determinants of health (SDoH)
  • Knowledge of medical terminology and care coordination
  • Call center or remote customer service experience
  • Strong interpersonal and communication skills
  • Organizational and time management abilities
  • Analytical skills for data collection and reporting
  • Familiarity with community-based services and social programs
  • Computer literate and good typing skills
  • EMR/EHR experience is preferred (Epic, etc.)
