This position is entirely remote, allowing for flexibility and the chance to work from the comfort of your own home while engaging with clients via telehealth systems and virtual meetings. • Provide direct clinical services from a remote setting, engaging with clients experiencing major mental illness and severe emotional disorders through virtual assessments, individual therapy, group therapy, and family counseling.
• Collaborate with other healthcare professionals to create and implement integrated care plans that ensure clients have access to necessary services and resources.
• Conduct brief, evidence-based screenings to ascertain clients’ needs and recommend further assessments as required, developing tailored therapeutic interventions based on best practices.
• Monitor client progress and adjust treatment plans accordingly, ensuring that services address presenting problems while empowering clients to actively participate in their care.
• Participate in multidisciplinary team meetings, contributing valuable insights on client care strategies and enhancement of service delivery.
• Assist with crisis intervention, providing timely support in acute situations to mitigate risks and enhance client safety.
• Maintain accurate and timely documentation of client interactions, treatment plans, and outcomes in compliance with organizational standards and ethical guidelines.
• Engage in ongoing professional development to stay current with best practices in mental health treatment and maintain licensure standards.

This role specifically targets a diverse clientele struggling with mental health issues within a hospice care framework. You will encounter individuals grappling with terminal conditions, anxiety, depression, and other complex emotional challenges. • A Master’s degree in Social Work from an accredited institution.
• Current and valid Licensed Clinical Social Worker (LCSW) certification in Colorado.
• Demonstrated expertise in mental health assessment, diagnosis, and treatment for diverse populations.
• Strong communication skills, both verbal and written, to effectively engage with clients, families, and healthcare professionals.
• Proficient in using electronic health records and telehealth platforms to document and deliver care.
• Ability to navigate complex care environments and foster relationships across various systems and organizations.
• Commitment to maintaining professional ethics and a focus on client empowerment.

• At least 2 years of clinical experience in mental health settings with a preference for hospice or palliative care experience.
• Experience working with a diverse range of clients with varying emotional and psychological needs.
• Background in crisis intervention and risk management strategies.
