Location: Office and Work-from-home Flexible along with North and East Bay Patient Visits
About the Role
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. This part-time, flexible, primarily remote position provides clinical oversight of skilled nursing services in accordance with:
California Title 22 HHA regulations and CDPH Home Health Agency standards
Accreditation Commission for Health Care (ACHC) standards
VA/TRICARE, Medi-Cal/CCS, and private insurance requirements
You will supervise field RNs/LVNs, guide clinical decision-making for both pediatric and adult cases, complete Home Health assessments, and ensure Atlas maintains excellence as we scale.
Responsibilities
1. Pediatric Clinical Leadership
Provide oversight and case management for medically fragile pediatric clients (vent/trach/enteral care, seizure management, CCS/EPSDT documentation).
Support transitions of care from NICU/PICU to the home.
Mentor nurses delivering long-term pediatric shift care.
Ensure compassionate, developmentally appropriate family communication.
2. Adult Clinical Oversight
Oversee care for adults with neurologic, respiratory, or mobility impairments.
Provide direction for veterans receiving continuous skilled nursing under VA/TriWest.
Review OASIS assessments, care plans, and documentation.
3. General Home Health Supervisory Duties
Ensure compliance with Title 22 RN supervision standards.
Create Home Health Plan of Treatment (CMS form 485) for Primary Care Physician approval
Conduct documentation review and chart audits.
Support QAPI and survey readiness.
Train and mentor RNs/LVNs.
Serve as clinical resource for changes in condition and escalations.
Qualifications
Active California RN license.
2+ years recent RN experience in pediatric home health, NICU, PICU, or high-acuity pediatric care (highly preferred).
Experience with adult home health or complex-care populations (e.g., tube feeding, trach care, ventilator management)
Strong clinical judgment for technology-dependent infants/children and medically complex adults.
Supervisory or charge experience preferred.
Home Health Plan of Treatment and OASIS experience preferred.
BSN preferred; PALS, CCRN, CPN, WOCN, or case management certification a plus.
