Position Summary

We are seeking an experienced RN Manager – Case Management to oversee the daily operations and clinical practice of the Case Management department at a respected acute care hospital in Santa Rosa, California.

This leadership role is responsible for managing Case Managers and support staff while ensuring effective care coordination, discharge planning, utilization management, and quality outcomes across the continuum of care. The manager will collaborate with physicians, nurses, social workers, and interdisciplinary teams to ensure patients receive timely, appropriate, and coordinated care.

Key Responsibilities

  • Provide leadership and oversight for the Case Management department.
  • Supervise, mentor, and develop Case Management staff.
  • Ensure effective care coordination, discharge planning, and transition of care.
  • Monitor department productivity, quality metrics, and regulatory compliance.
  • Lead quality improvement initiatives and performance management activities.
  • Manage departmental budgets, staffing, recruitment, onboarding, and retention.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and resource utilization.
  • Ensure compliance with organizational policies, accreditation standards, and regulatory requirements.

Required Qualifications

  • Bachelor's degree in Nursing (BSN).
  • Active California Registered Nurse (RN) license.
  • Minimum 5 years of healthcare experience.
  • Minimum 3 years of Case Management experience in an acute care/inpatient hospital setting.
  • Minimum 1 year of healthcare leadership or management experience.

Preferred Qualifications

  • Master's degree in Nursing, Healthcare Administration, or a related field.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM).
