21st Century Home Health Services Inc. is seeking a Full Time Registered Nurse Case Manager, Level 2, to cover Sebastopol and surrounding areas. The RN will lead two nurses, develop and implement care plans, and coordinate with physicians and an interdisciplinary team to prevent rehospitalizations. Qualified candidates will hold a CA RN license, BLS/CPR, and have Home Health Case Management experience. A valid CA driver's license is required with reliable transportation to patient homes.
