Key Responsibilities
  • Conduct thorough assessments of clients upon intake to identify their needs, strengths, and treatment goals.
  • Develop personalized treatment plans using a solution-based approach that aligns with best practices and the specific circumstances of each client.
  • Provide a variety of therapeutic interventions, including individual and group therapy sessions, tailored to support client recovery.
  • Regularly evaluate and adjust treatment plans based on client progress and feedback, ensuring dynamic and responsive care.
  • Maintain meticulous, confidential records of client interactions, assessments, and treatment outcomes in compliance with HIPAA regulations.
  • Collaborate with a multidisciplinary team of professionals to enhance client care, sharing insights and strategies that contribute to improved patient outcomes.
  • Stay informed on current trends, research findings, and best practices in social work and mental health to continually elevate the standard of care.
Patient Population / Client Focus

You will primarily work with a diverse client base, including individuals overcoming trauma, experiencing anxiety, and managing depression. Each client's journey is unique, and your role will be pivotal in providing focused and empathetic support that encourages personal growth and resilience. At our facility, we foster a holistic view of recovery, integrating physical, emotional, and psychological aspects to ensure comprehensive care for all clients.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • An active LCSW license in New York state is mandatory for this role.
  • Strong interpersonal and communication skills to build connections with clients, team members, and stakeholders.
  • Ability to navigate complex emotional situations with patience and professionalism.
  • Demonstrated proficiency in clinical documentation and adherence to ethical practices.
Preferred Experience

While a minimum qualification is a Masterโ€™s degree and an LCSW license, candidates with prior experience in rehabilitation settings, particularly working with clients who have experienced trauma, anxiety, or substance use disorders, will be highly regarded.
