Key Responsibilities

• Conduct thorough assessments and provide counseling services to individuals and families, including children, youth, and adults, addressing a variety of mental health concerns.
• Collaborate with the Practitioner team to identify new risks and de-escalate potential crises, ensuring that appropriate interventions are enacted swiftly and efficiently.
• Manage case documentation accurately and ensure compliance with ethical and legal standards, specifically in relation to mandatory reporting and safeguarding procedures.
• Play a lead role in escalating issues to emergency services as necessary while maintaining clear communication with clinical leads regarding risk assessments and potential safeguarding threats.
• Keep abreast of the latest policies and procedures regarding client safety, actively promoting best practices within our service delivery models.
• Engage in outcome-focused practices by collecting detailed feedback from clients, analyzing barriers to care, and contributing to service improvement recommendations.

Patient Population / Client Focus

The LCSW will work with a diverse range of clients, including individuals grappling with mental health issues, substance use/abuse challenges, and those in need of palliative care support within a hospice setting. Qualifications

• A Master’s Degree in Social Work from an accredited graduate program is required.
• A valid California Licensed Clinical Social Worker (LCSW) credential is mandatory prior to employment.
• A minimum of two years’ experience in a behavioral health setting, providing direct services to clients.
• Demonstrated proficiency in conducting psychosocial history assessments, case management evaluations, and suicide risk assessments is essential.

Preferred Experience

• Experience working with diverse populations, particularly in hospice or end-of-life care settings.
• Proficiency in electronic health record systems and telehealth platforms.
• Skills in conflict resolution and mediation within clinical contexts, particularly with sensitive client situations.
