• Conduct comprehensive psychosocial assessments to understand and address the emotional, social, and financial challenges faced by our patients.
• Facilitate both individual and group counseling sessions tailored to help patients manage adjustment issues, grief, and coping strategies.
• Collaborate closely with multidisciplinary medical teams, including physicians and nurses, to provide a holistic approach to patient care and improve healthcare outcomes.
• Develop and implement effective discharge plans that ensure patients are connected to community resources for ongoing support after leaving our clinic.
• Participate in crisis intervention situations, providing immediate support and guidance to patients and families experiencing acute distress.
• Educate patients and families about treatment options and available resources to empower them in their recovery journey.
• Maintain up-to-date and accurate patient records in compliance with ethical and legal requirements, using our electronic health record systems.
• Engage in continuous professional development by participating in committees and contributing expertise in social work-related matters.

The Licensed Clinical Social Worker will primarily serve individuals within the community health clinic framework, providing care to a diverse range of patients facing various psychological challenges. This includes those experiencing chronic illnesses, mental health disorders, and socio-economic challenges. You will play a vital role in enhancing patients’ recovery experiences and overall well-being by addressing their psychosocial needs comprehensively.

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

• At least two years of direct experience working in a healthcare or hospital environment, providing social work support to patients.
• Proficiency in understanding medical terminology and mental health disorders, along with crisis intervention techniques.
• Demonstrated skills in communication, collaboration with interdisciplinary teams, and a compassionate approach to patient care.
