Lehigh Valley Companion Care Homes is seeking a compassionate and dedicated Registered Nurse (RN) to join our team on a PRN basis. The ideal candidate will deliver high-quality in-home and community-based care to clients, ensuring their health and well-being needs are met in a comfortable and nurturing environment.


Responsibilities:

  • Provide skilled nursing care to patients in their homes and in the community setting.
  • Administer medications and monitor treatment plans as prescribed.
  • Perform health assessments, including vital signs, and document findings.
  • Assist with personal care tasks, as needed.
  • Support patients with mobility and physical therapy exercises to enhance their overall well-being.
  • Ensure patient safety and comfort through proper care and supervision.
  • Coordinate with family members and healthcare providers to ensure optimal care and support.
  • Respond to medical emergencies as needed and provide first aid as required.
  • Document all care provided and maintain accurate patient records to ensure comprehensive and up-to-date documentation.


Qualifications:


  • Current and valid Registered Nurse (RN) license in the state of Pennsylvania.
  • Minimum of 1 year of nursing experience in home care, long-term care, or similar healthcare settings preferred.
  • Ability to work independently and manage patient care responsibilities.
  • Compassionate and patient-centered approach to care.
  • Strong communication skills and the ability to work well with clients, families, and other healthcare professionals.
  • Ability to handle the physical demands of the job, including lifting and moving patients as needed
  • Ability to work on an as-needed (PRN) schedule
