Job description

RN needed full time or PRN for home health visits.

Home health and Kinnser / Wellsky experience a plus!

Coverage areas: Desoto and surrounding areas

Key Responsibilities

Patient Care: Administer nursing care to patients in their homes, including wound care, medication administration, and monitoring vital signs.

Assessment: Conduct thorough assessments of patients' health conditions and document observations to inform care plans.

Care Coordination: Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.

Patient Education: Educate patients and their families about health conditions, treatment plans, and home care procedures to promote independence and recovery.

Documentation: Maintain accurate and timely records of patient care, progress, and any changes in condition.

Qualifications

Education: A minimum of an Associate's Degree in Nursing (ADN) or a Bachelor of Science in Nursing (BSN) is required.

Licensure: Must hold a current and unencumbered RN license to practice nursing in the state of Texas.

Experience: Previous experience in home health care or a similar setting is preferred as us bilingual!

Skills: Strong clinical skills, excellent communication abilities, and a compassionate approach to patient care are essential.
