Job description

Summit Home Health Care is currently hiring New York State Registered Nurses to perform tele-health UAS Assessments. Training will be provided. We currently have multiple opportunities for per diem earning fee for service. We are currently paying $130 per UAS assessment. 

 Responsibilities of the RN

  • Assess eligibility of Medicaid members in need of managed long-term care using a comprehensive web-based system.
  • Conduct initial and annual assessments remotely, according to state guidelines, policies and protocols.
  • Consult with health care providers, and review medical records and medications to support assessment outcomes.
  • Review and revise assessments as necessary based on company policy.

 Requirements of the RN

  • New York State Registered Nursing (RN) license.
  • New York State Drivers License or New York State Identification Card (if living out of NYS).
