This is a fee-for-service position. In order to be considered for this job you MUST have a valid RN license in the state of NY and prior UAS experience.

Rates are paid per visit.

Pay: Initial visit- $165.00 per visit; Reassessment- $140.00 per visit

Job Summary: We are seeking a dedicated and compassionate Nurse to join our healthcare team. Responsibilities:

  • Conduct UAS assessments for clients to determine their level of care and support needs.
  • Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately.
  • Provide compassionate and professional interactions with clients in various community settings.
  • Submit completed assessments within designated timeframes and maintain detailed records.
  • Collaborate with other care providers to ensure a comprehensive assessment process.
  • Attend required training sessions and stay informed of assessment updates.

Qualifications:

  • Registered Nurse, currently licensed and registered to practice in New York State.
  • UAS-NY Certified
  • Bachelor's degree in nursing preferred.
  • Experience in Home Care, case management, or long-term home care.
  • Demonstrated skills in planning, organizing and directing the work of others required.
  • Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.
  • Proficient with DOH UAS assessment instrument.

License/Certification:

  • BLS Certification (Preferred)
  • RN License (Required)
